SARMs vs. Anabolic Steroids: Evidence, Risks, and Canada Status

Short answer: current evidence does not support calling selective androgen receptor modulators (SARMs) a safe or approved alternative to anabolic steroids. The two groups differ in chemistry, research history, and regulatory treatment, but both can affect androgen signalling and both carry important uncertainties. Direct head-to-head human safety trials are not available, so a simple “which is safer” ranking would overstate the evidence.

This research summary explains what is known, what remains uncertain, and how the Canadian regulatory position should shape interpretation. It does not provide dosing, cycle, or human-use guidance.

What human SARM research can and cannot tell us

A 2023 systematic review identified 33 eligible reports involving clinical trials and case reports. It found liver-enzyme elevations in trials and case reports involving drug-induced liver injury, tendon rupture, and rhabdomyolysis. These data identify safety signals, but they do not provide a complete long-term risk estimate.

A later sports-medicine systematic review also emphasized the limited quality of evidence available for athlete use, including contamination concerns and reports of liver injury, myocarditis, and tendon rupture. Clinical findings from controlled investigational products should not be assumed to describe products obtained from an unverified online supply chain.

Health Canada’s current bodybuilding-products guidance is more direct: SARMs are not authorized for medical use in Canada, and products sold online may contain a different drug than the label claims. That makes identity and documentation separate questions from the pharmacology of the named compound.

Why “safer” is the wrong conclusion

Mechanistic selectivity is a research hypothesis, not a consumer safety designation. A compound may behave selectively at one exposure or endpoint and less selectively at another. Short studies can also miss delayed, uncommon, or cumulative outcomes.

For anabolic steroids, recognized risks include cardiovascular, hepatic, endocrine, reproductive, and psychiatric effects. For SARMs, the evidence base is smaller, but smaller does not mean safer: it means the uncertainty is larger. Without direct comparative trials using defined products, comparable exposures, and adequate follow-up, a categorical safety ranking is not scientifically defensible.

Canada-specific regulatory context

Health Canada says that SARMs have not been authorized in Canada for any medical use. Authorized prescription drugs carry an eight-digit Drug Identification Number (DIN); a research-use label does not convert an unauthorized health product into an approved medicine. Anabolic steroids and their derivatives are separately regulated as controlled substances.

How to evaluate research claims

  • Confirm the compound, formulation, concentration, lot, and analytical method.
  • Separate cell and animal findings from controlled human outcomes.
  • Check the population: oncology, frailty, or other clinical cohorts do not represent healthy recreational users.
  • Look for duration, comparator, adverse-event reporting, withdrawals, and laboratory monitoring.
  • Do not treat a vendor’s “research use only” wording as evidence of regulatory approval or human safety.
